HELP US HELP YOU – INSTRUCT DRIVERS AND STAFF TO:

Promptly report all losses to us by phone. Follow-up by sending the accident
report, etc. via fax. (Promptly meaning taking action without delay.)

Instruct drivers to report ALL accidents or incidents from the scene where
possible. Remember that low speed, low impact, minor or no visible damage
loss incidents not reported often develop into a claim catastrophe.

Make sure we are given every phone number available. Truckers are naturally
hard to reach. We need cell, beeper, fax, home, e-mail, office, etc. numbers.
Be sure to get home, work, cell, beeper, e-mail, etc. numbers of other parties
involved, witnesses, police, etc.

All physical damage losses to insured equipment must be reported before
repairs have begun.

All lawsuits, legal representation notices, claimant inquiries, request for filing
SR-21 forms, and subrogation notices, etc., are to be forwarded immediately
via “overnight mail” to the attention of the home office Claims Department,
